If you weren't 100% sure that the sister was dead, I feel like I should clarify a few things so that you don't go looking for a discount surgeon online.
Transplants cannot be performed- In a garage
- By one person
- By abducting a random person and hoping their blood type matches that of the person receiving the transplant
- With kitchen knives, no matter how nice the set is
- Without any kind of medical equipment keeping the 'patients' alive/breathing
- By a teenager whose medical training consists of "well I pulled a lung out of a dead bird one time so I think I've got this down"
Please avoid any craigslist surgery ads if you feel any of these factors might be involved!! But seriously, the sister is very, very dead.

are you even being serious with that? You don't have to be a doctor to be familiar with heart-lung machines.
edit: oh, were you poking fun at me bc I referred to a "breathing" machine for someone with no lungs in their body? lol, it was obvious what I meant. -

Maybe a few people thought it was going the way of this film's sibling-in-spirit MAY, where (Spoilers for MAY)

But they didn't show anything like that. If they had shown the sister seeming to come alive in Pauline's mind, I could understand how people might be a bit confused. But as the OP said, it's pretty clear that Grace, the nicest and most innocent character in the whole cast, is dead.
It actually was a pretty shocking ending. I was certain Traci Lords would be the one to die.

Agree with you DHB - whats so complicated??? An 18 year old complete whack job has killed two people because she has convinced herself she is a surgeon and can actually perform a lung transplant. Pretty simple. I think the only question for debate is Pauline's reaction when her mother grabs her. In any event, terrific film. Watched it again last night - INTENSE!!

I think the blood coming out of her sister's mouth during the procedure was probably a bad sign. Having airways full of blood is going to be an issue when you transplant new lungs. Also she didn't seem to be sufficiently concerned with getting her sister breathing again once she'd finished the operation.

What grinds my gears: people who misuse the word pedophile. Get some education. A pedophile is someone sexually attracted to PREPUBRSCENT kids. A 14 year old is NOT prepubescent. Pedophiles are attracted to kids below the age of 10-11.
