She has such a break down by the end of the film that the doctor explains that she's now in 'the quiet room' meaning they have to sedate her because she's violent and uncontrollable. She has no moments of lucidity anymore, so she can't tell anyone who the killer truly was.

Just guessing here, but they might have set it in the 90's because most mental illnesses werent recognized as a serious disease truly until the 80's and 90'sand even then, people with the illnesses were treated with shock therapy and risky brain surgeries that were trial and error instead of meds and therapyscary world to live in, the 60s with a mental illness. Habut yeah, thats my guess anyway.

I love the ending. For me this movie stands alone apart from any basis in real events. They should have just said 'inspired'. I loved the entire movie. I didn't realize until I saw this that Eliza Dushku was that great an actress. The writing was sharp and realistic in my opinion. This is one of the rare movies where I felt the horror the character was feeling.

When they say "based on" audiences think it really happened that way. When they say "inspired by" most people realize that it is pure fiction.

Even if they believe her (the cat and the fact that he knew all the victims are solid clues to back up her story), they will probably assume he didn't make it.
Falling into cold, moving water, with at least one gunshot wound, his odds were not good.
The images of him in a church (sitting on a bench, not in his wheelchair + with another priest and another setting than the first church) also suggest that he moved elsewhere and assumed another identity .

The very end of this this was very dissatisfying because it concludes on this frustratingly anticlimactic cliffhanger note. Plus the big reveal of the killer's true identity was awfully damn absurd and preposterous.
You've seen Guy Standeven in something because the man was in everything.
