A more slight built actress would have haved to bulk up for it to work, like Rebecca DeMorney did in "Hand that Rocks the Cradle", DeMorney was too pretty to play Wilkes but perhaps Renne Russo could have?
Midler's cheeriness would have worked well for the 'sunny' side of Annie; she also does gloomy and manic really well, remember her in "Ruthless People"? -

Anjelica Huston, Tyne Daly and Kathleen Turner are the three best alternate casting choices imo. But Kathy Bates was the perfect choice in the end.
