But Jed worked as Dr. Lillianfield at that abortion clinic. Tracy's mom tells him how she got got an abortion at a clinic, and ended up working for the Dr. there. Then she made off with eighty thousand of the clinic's money. And Andy say's "Was the name of the doctor david lillianfield"? And she says yes. So why was he practicing at a clinic under a fake name? Jed is his real name. Why not use it???

What part don't you understand? Jed didn't ever pretend to be Lillianfield. There was only an answering service for Lillianfield. There was a REAL Lillianfield Tracey bilked out of 85K in the past but Jed never pretended to be him.

There was never a real Lillianfield. Lillianfield WAS Jed Hill. At the bar Jed asks Andy who Tracy is seeing and Andy says Lillianfield in Boston. Jed nods and says Lillianfield's a good man. She was obviously with Jed all the time she told Andy she was getting treatments in Boston. We know Jed was injecting her with the drugs. Jed has that fancy house on the cliffs, the answering service lists it as the address of Lillianfield. Don't you see??? It makes no sense why he used an alias. And when he used that alias of Lillianfield at the abortion clinic that's where he met Tracy. Don't get me wrong, I love this movie, seen it probably twenty times, can practically quote it word for word. But, that Lillianfield thing will always be a mistake that wasn't thought through by the writers.

There really are two possible explanations:
- Hill meets Tracy before the abortion clinic and when her first scam with the guy she was trying to marry doesn't work she comes up with a new scam and she brings hill in on it. Hill pretends to be Lillianfield in the abortion clinic in order to help her swindle the clinic out of $80,000 and then they decided to move on to a bigger score. Hills own star begins to rise and as it does there becomes an opportunity to cash in. They use his old alias to set up a fake doctors office but since you need a real address to be billed for the answering service they have to use his real address. Then their plan goes into motion.
- Lillianfield was really some poor sap of a doctor in the abortion clinic that Tracy took for all he was worth. She meets Dr Hill and comes up with the new scheme to take the hospital for millions. She has to come up with a name of a doctor so she uses the name of the guy she duped years ago. The answering service needs to be used to make the doctor seem real just in case her husband calls the number, to buy the answering service you need to have an address attached to it so why not use the address of your accomplice Dr Jed Hill. Jed knows this, he's in on the plan so when asked he lies and says "Lillianfield, good man." When he has never even met the real lillianfield nor does he know if he even really exists.
It's not a mistake just kind of weak, and vague with plenty of holes in the story line.

Only the first time I've seen it, oddly, but at some late point in the film when Hill and Tracy are arguing, doesn't he say something like "Once Andy had found out he was sterile, how long did you think it would take him to realise that there was no Dr Lillianfield"?

I agree with others that there was a Dr. Lillianfield she swindled earlier and they just used that name. Baldwin saying "how long did you think it would take him to realise that there was no Dr Lillianfield?" is talking specifically about the one they made up whom Andy would br tracking down, not the real Dr. Lillianfield.

But in Tracie's medicine cabinet there are prescriptions from Dr Lillianfield. So, he is real in some senseeither Jed perpetuating a huge fraud or he's real but why would he prescribe meds for Tracie after she robbed his clinic of 80K?

The nurse neighbor did not leave her son home alone. That was a con, a setup for Tracy. The police were working with Andy and the nurse neighbor. We don't actually see them concocting the setup, we conclude this happened when we see Dana the cop come in near the end, dressed in a nurse costume.
Dr. Lillianfield: We know from Tracy's mother that Dr. Lillianfield was a doctor working at an abortion clinic where Tracy worked. Tracy stole $80,000 from this clinic. My assumption is that Tracy also stole Dr. Lillianfield's identity. She and Jed used this identity to write prescriptions (we see them in Tracy's medicine cabinet) and to set up an account with an answering service. Jed and Tracy needed an alias doctor as a part of their scam. -

The nurse neighbor did leave her son home alone, every night she had to work. Early in the movie, we see her exiting the home in her uniform, calling up the stairs, "I want you in bed by ten!" She wouldn't have said that if he had a sitter or some other adult there.
It's a touch ambiguous if this is a five-days-a-week routine, or just Mom's Occasional Night Shift, but I think it's strongly implied by the fact that the kid plays his piano at night a lot (Tracy & Andy complain about it), and the fact that nurses (and most other shift workers) tend to be night shift or day shift almost exclusively; people aren't scheduled to work eight-pm-to-eight-am and then expected to come back at eight am the next day. Often there's a shift pay differential between night shift & day shift, too, so they don't switch back and forth much. Last, if there had been a babysitter even occasionally, Tracy wouldn't have been as confident breaking into the house to kill the boy (breaking a window, no less, which makes noise–but Tracy knows the kid won't hear it, and she knows there's no one else in the house to hear it, either).
So yes, that last night was a con setup for Tracy, but it wouldn't have worked if Tracy hadn't been familiar already with the nurse's normal routine and the fact that the kid was left alone. -

Off the top of my head. Abortion clinic means the possibility of right wing right to life nutjobs stalking or murdering doctors who perform them. These things have been escalating since the late 1970s.
Perhaps it's possible to operate in that field of medicine with a public assumed name but with your real name known only to the state licensing board. A quick google turns a California application for operating under an assumed name. -
