    Yes it is vague and weak. Another weak point is the fact that Tracy didn't know her next door neighbour's boy was blind. I know people live isolated lives these days, but that is ridiculous.

      It was believeable that Tracy didn't know the neighbour boy was blind. She is a sociopath/antisocial and they can actually be very self-centred and unobservant.

            I agree with others that there was a Dr. Lillianfield she swindled earlier and they just used that name. Baldwin saying "how long did you think it would take him to realise that there was no Dr Lillianfield?" is talking specifically about the one they made up whom Andy would br tracking down, not the real Dr. Lillianfield.

              But in Tracie's medicine cabinet there are prescriptions from Dr Lillianfield. So, he is real in some senseeither Jed perpetuating a huge fraud or he's real but why would he prescribe meds for Tracie after she robbed his clinic of 80K?
              Even if it means me taking a chubby, I will suck it up!

We don't actually see them concocting the setup, we conclude this happened when we see Dana the cop come in near the end, dressed in a nurse costume.
                Dr. Lillianfield: We know from Tracy's mother that Dr. Lillianfield was a doctor working at an abortion clinic where Tracy worked. Tracy stole $80,000 from this clinic. My assumption is that Tracy also stole Dr. Lillianfield's identity. She and Jed used this identity to write prescriptions (we see them in Tracy's medicine cabinet) and to set up an account with an answering service. Jed and Tracy needed an alias doctor as a part of their scam.

                  The nurse neighbor did leave her son home alone, every night she had to work. Early in the movie, we see her exiting the home in her uniform, calling up the stairs, "I want you in bed by ten!" She wouldn't have said that if he had a sitter or some other adult there.
                  It's a touch ambiguous if this is a five-days-a-week routine, or just Mom's Occasional Night Shift, but I think it's strongly implied by the fact that the kid plays his piano at night a lot (Tracy & Andy complain about it), and the fact that nurses (and most other shift workers) tend to be night shift or day shift almost exclusively; people aren't scheduled to work eight-pm-to-eight-am and then expected to come back at eight am the next day. Often there's a shift pay differential between night shift & day shift, too, so they don't switch back and forth much. Last, if there had been a babysitter even occasionally, Tracy wouldn't have been as confident breaking into the house to kill the boy (breaking a window, no less, which makes noise–but Tracy knows the kid won't hear it, and she knows there's no one else in the house to hear it, either).
                  So yes, that last night was a con setup for Tracy, but it wouldn't have worked if Tracy hadn't been familiar already with the nurse's normal routine and the fact that the kid was left alone.

                    Off the top of my head. Abortion clinic means the possibility of right wing right to life nutjobs stalking or murdering doctors who perform them. These things have been escalating since the late 1970s.
                    Perhaps it's possible to operate in that field of medicine with a public assumed name but with your real name known only to the state licensing board. A quick google turns a California application for operating under an assumed name.
