I know the book is the book but I'll say something about how it was in the book, without any spoilers.
By way of bullet points:- Vera was never one to heed medical advice
- Vera had never been declared mentally incompetent, so she was very much in control of where she lived
- Vera had additional part-time staff so it can be reasoned that she'd pay for the island doctor to visit her on a regular basis
- Dolores had proven herself to be very competent, by way of scarcely falling foul of Vera's nutty, dictatorial and highly specific rules
- Vera had grown to love Dolores as her dearest friend and only Dolores comforted her during Vera's night terrors
- The story takes place on Little Tall, an island that in Stephen King's universe has a strong islanders' code of staying on the island. Vera has always loved Little Tall so she chose to move there permanently before she fell ill. There's no way she would leave
So all in all, Dolores was probably the best placed person to care for Vera.

Yikes! I don't think a nursing home would have coped with Vera. It was perhaps a godsend for both nursing staff and other residents that she wasn't placed in one. Besides, Dolores had a comfortable roof over her head by being Vera's aid, and this was something she ended up acquiring for herself.
She took better care of Vera than anyone else could and she could read that snake like a book.

Terrific! I love both the novel and the movie. The novel is more intimate in nature, but the film still gets a fine balance by expanding the story to give us Selena's tale and side of the story too.

I have read most of King's novels up until the early 90's. The last one of his I read was the late 90's,
The Green Mile
.
Dolores Claiborne
I have read over and over again. It is one fine story and this is about real life's horrors and sufferings without all the supernatural trappings.

I read part of The Green Mile but I don't think I finished it, probably skimmed
