Why can't vaginas be shown in R-rated movies?

There are plenty of films with Kate Winslet completely naked and they are rated R. In Jude, you see the lips underneath all the hair (laughed at the covered by hair conspiracy).

As a gay man it is weird I've not seen a vagina or know how one works. No sex ed. No vagina in movies.

Streaming services like hbo are not held to the same mpaa standards yet they do not show female genitalia and have no problem showing male genitalia. I wonder why?
