Unique voice

Mr. Fierstein has one of the most unique voices in show business, but I'm wondering how that voice came to be. Did he suffer a throat injury in his past? I looked in his IMDB bio and couldn't find anything

From what I understand, very early in his career he was in a play that required him to deliver a long monologue over the top of a gospel recording of "God Bless America" that was being played very loudly. Harvey, ever the dedicated professional, wanted every word of the monologue heard, and over the course of the season he did permanent damage to his voice, resulting in the iconic rasp that is now his trademark.

I've been wondering about his voice for years because I know he's sounded like that for a long time and wasn't sure if that was just a character voice he did (like Gilbert Gottfried), but can't talking like that constantly cause polyps or some other detrimental problems?

His trademark voice is the result of over-developed ventricular folds in his throat, which produce a hoarse-sounding double voice.
"
He's quoted as saying that "the average voice is about 70% tone & 30% noise," whereas his is more like 95% noise.
